state department continues phase-in of new nonimmigrant visa application form

The U.S. Department of State is continuing the phase-in of its new Nonimmigrant Visa Application form, Form DS-160. The new form incorporates several previous forms, such as the DS-156, DS-157 and DS-158 and combines them into one form which will be used for all nonimmigrant visa applications.

Further, please note that the State Department has mandated that numerous additional Consulates begin using the new form no later than March 1, 2010. This includes the following Consulates:
